If you have decided that CCI is your child-placing agency, the first step in your adoption journey is to download and complete the CCI application for adoption.

Items to submit with application:
- Five reference forms
- For families new to CCI, please use this form.
- Returning CCI families, please use this form.
- Up to two of the reference forms can be replaced with notarized reference letters (you may provide us with copies of these notarized letters and keep the originals for your dossier). At least three of the five references must use the reference form.
- No more than two relatives can complete references.
- $250 fee must accompany application to begin processing, unless application is also for home study. See Florida Residents Only section for more information. Fee is non-refundable.
- Recent family photo
- Copies of prospective adoptive parents’ passports, if available.
- Singles Only: Statement of heterosexuality. Please do not change any wording except name and notary section information.
- Notice of Non-Preferential Treatment of Donors form
- Ethiopia Program Only: You must submit this addendum to the application
- Ethiopia Program Applicants: If you do not already have a passport, we highly suggest you apply for one now. You will need a copy of your passports in order for a case to be submitted to the Ethiopian courts.
Remember, ALL information provided will remain CONFIDENTIAL and will only be used by CCI personnel, your home study provider and others as necessary to complete your adoption. It is essential that you provide full disclosure regarding the application's questions. Failure to provide accurate and complete information will prevent CCI from processing the application.
